I have a Remington R15. It's about 4 years old with 1000 or so rounds through it. Lightly used and well maintained for the most part. I believe it's been happening for a while, but if I chamber a round and then say, unload it, the round comes out with a dented primer. Obviously not enough to fire it, however it's a decent little dent.
Is this a slam fire waiting to happen? Need a new powder to replace RL-15 > Reloading > AR15.COM. 77 grain TMK loads > Reloading > AR15.COM.

I ended my post with the comment how different measures do better or worse with different powders.
